Project Homeless Connect

At a Glance

Mission Statement:
San José Project Homeless Connect (PHC) seeks to be an integral resource in providing services and information to citizens of Santa Clara County who are trying to regain or maintain permanent housing.

The Event:
PHC is a coordinated effort to provide the broadest possible array of health and human services on one day, in one location. Community volunteers partner with local non-profit service providers, businesses, faith-based organizations, and the government to connect our homeless residents to medical services, mental health programs, dental providers, housing options, SSI benefits, employment resources, legal counseling, California IDs, food, haircuts, and much more.

The Evolution towards Better Service:
In light of PHC’s ability to lower barriers to access, expedite outcomes, and provide convenience and choice for clients, in 2009 the City of San José moved forward with creating two permanent One-Stop Homelessness Prevention Centers at InnVision’s Georgia Travis Center and EHC LifeBuilders’ Boccardo Regional Reception Center. At these locations, the general benefits of PHC are available on a daily basis. Therefore, by now focusing PHC on more geographically peripheral areas of Santa Clara County, we hope to connect even more homeless and at-risk residents to the housing and services they so desperately need.
